In an effort to be more paperless and efficient, we are requesting that our insured's be more proactive in obtaining email addresses for the delivery of COI's to both the Certificate Holders and to the Insured's.  Those holders or inured's that do not use email can still be sent copies via fax and US Mail.  Your efforts in this regard will help us to provide a better electronic paper trail for when COI's are sent as well as making sure that you, the insured, get a copy of each COI that is processed. 

If you prefer to fax your request, please print the COI Fax Form and submit your request via the fax number on the form.
